No Surprises Act Provider Open Negotiation Information
Employee Benefits Logistics administers self-funded group health plans subject to the federal No Surprises Act.
Providers and facilities who wish to initiate open negotiation regarding an out-of-network payment determination must submit a written request within 30 business days of receipt of the applicable initial payment or notice of denial of payment.
Open negotiation requests should include:
Patient name
Member ID
Date(s) of service
Claim number
CPT/HCPCS codes
Billed charges
Contact information for the provider/facility representative
